To assist in fully meeting your needs, you may utilize a combination of both Self-Administered Services and Provider Agency Services. Contact your Support Coordinator for more information.
To be eligible to participate in Self-Administered Services, persons with disabilities must qualify for the services listed below, as determined by their Support Coordinator. These services are also available under the Provider Agency Services program, with the exception of Personal Assistance:
