- Support individuals in any outcomes and desires that might be addressed in their Person Centered Plan.
- Client intake, assessment of community resources needed the functional skills of the individual and what assistance they would require including family and natural support persons.
- Advise on obtaining and identifying community resources such as Social Security, social services, mental health, school programs, housing, facilitate financial assistance, guardianship, work incentives programs, may include information on state and federal programs.
- Help with developing and implementing links for client within the community.
- Development and facilitation of natural supports and teaching persons to effectively use and maintain them.
- Promoting involvement with professionals, co-workers, natural supports, educators and social activities within the community.
- Advocate issues; assisting with specific discussions or meetings with support Coordinator_ and Rehabilitation Counselors on behalf of the person; and facilitating and or/ maintaining natural supports; Vocational assessment including interest exploration and planning; Job Development and Placement : Locating potential employers in the community, introducing the person to specific employers; conducting job analysis; arranging for any certifications needed.
CHOICE Supported Employment Services, Inc. has over Twenty-five years of experience in the Human Service field, currently contracted with DHS/DSPD and DRS providing supported employment services for many individuals in need of work. CHOICE has worked closely with several Support Coordinators and Vocational Rehab Counselors in the Weber and Davis counties in providing supported employment services, they are aware of our service capabilities to provide all components of the Community Service Broker Model.
